Next, you need your proof of identification – This is usually a copy of your driver’s license or state or military issued ID card.
Then you need to get two passport photos – 2″x2″ Identical photos to include on your passport.
Once you have that, just fill out the proper form. For new passports, fill out form DS-11. If you are renewing your passport, it is form DS-82. And if your passport happens to be lost, stolen or damaged, then you need to fill out both forms DS-11 and DS-64.
